The Blues defied the odds to reach the Carabao Cup semi-final after trailing Toon for 74 minutes.
Callum Wilson was gifted an opener for the visitors by a horror error from Benoit Badisahile, whose bad touch put the striker through on goal.
Chelsea huffed and puffed but were largely wasteful in attack until Mudryk popped up with a late equaliser.
Kieran Trippier failed to clear a cross to the far post and the winger pounced to fire past goalkeeper Martin Dubravka.
The match went to penalties at the end of the 90 minutes and Matt Ritchie’s decisive penalty was saved by Đorđe Petrović to send Chelsea through.
Trippier’s miserable night also continued as he fired wide from the spot.
